If you are assertive, ask questions, read about your own illness/ medical problems, you will have less of a chance to be admitted to a hospital, or get readmitted after being discharged from one.If your family member gets discharged from a hospital for a surgery or a medical reason, getting more informed, confident, and asking more question of both the nursing and medical staff before discharge will decrease complications for your loved one as well. If skills are needed to facilitate recovery from a procedure or medical admission, be proactive, and ask your physical therapist, clinician, or nurse about how to best help yourself in maximizing your role in your recovery. Handoffs between transitions of care can be dropped, resulting in substandard care, when doctors or nurses don\’t talk to each other clearly enough regarding your medical needs. So you have to take the initiative often and be proactive in making sure you understand, and your primary care doctor understands, what has happened to you, and what to expect in the future.
